1. The Contract Between Us


These Terms & Conditions form a legally binding contract between:


JG DERMATOLOGY LIMITED (“the Company”, “we”, “us”, “our”)

and

You, the patient (“you”, “your”)

JG DERMATOLOGY LIMITED provides private medical services through Dr Joanna Gach.


By booking and/or attending an appointment, you confirm that you have read, understood, and agree to these Terms & Conditions.


This contract applies whether you are self-funding or insured.


2. Our Services


“Our Services” means private dermatology services provided by JG DERMATOLOGY LIMITED through Dr Joanna Gach, within the scope of her clinical practice, including but not limited to:

  • New patient consultations
  • Follow-up consultations
  • Review appointments
  • Diagnostic assessments
  • Minor in-room procedures (e.g. injections, cryotherapy, biopsies)
  • Clinical advice and treatment planning
  • Prescriptions
  • Medical letters and reports

Our Services do not include hospital facility services, theatre services, nursing services, laboratory services, or pharmacy costs. These are provided and charged separately by the relevant provider.


3. Independent Practitioner Status


Dr Joanna Gach provides clinical services through JG DERMATOLOGY LIMITED as an independent Consultant Dermatologist.


Although consultations may take place at private hospitals or clinic facilities, neither Dr Joanna Gach nor JG DERMATOLOGY LIMITED is employed by those hospitals.


Professional fees charged by JG DERMATOLOGY LIMITED are separate from and in addition to any hospital or clinic charges.


4. Fees and Payment Terms


4.1 Consultation Fees


Consultation fees apply to all appointments, including new and follow-up consultations.


The current fee schedule is available on request and/or published on the practice website in accordance with fee transparency requirements.


4.2 Procedures and Additional Fees


If a procedure is performed during your consultation (including but not limited to injections, cryotherapy, biopsies, or minor surgery), additional professional fees will apply.


Where applicable, separate charges may also apply for:

  • Histopathology/laboratory analysis
  • Consumables
  • Hospital or facility fees

4.3 Payment Timing


Payment must be made:

  • In advance of the appointment; or
  • At the latest, on the day of the appointment

If payment has not been received in advance, a paper invoice will be issued at the end of the appointment and must be settled immediately.


We reserve the right to withhold further appointments, reports, results, or treatment until outstanding balances are cleared.


5. Self-Funding Patients


If you are self-funding, you are responsible for the full cost of:

  • Consultations
  • Procedures
  • Investigations
  • Histology/pathology
  • Prescriptions
  • Any other services provided as part of your care

Full payment remains your responsibility regardless of any potential reimbursement from an insurer.


6. Insured Patients


If you are using private medical insurance:

  • You must obtain valid pre-authorisation before your appointment.
  • You must provide membership and authorisation details in advance.

You remain personally responsible for all fees charged by JG DERMATOLOGY LIMITED.


This includes:

  • Policy excesses
  • Shortfalls
  • Non-covered services
  • Benefit limits
  • Any insurer refusal or partial payment

JG DERMATOLOGY LIMITED invoices patients directly. It is your responsibility to seek reimbursement from your insurer where applicable.


Late cancellation and non-attendance fees are not recoverable from insurers and remain payable by you.


7. Cancellation Policy


£200


Notice must be received during working hours. Communications received outside working hours will be deemed received the next working day.


8. Non-Attendance


£200


This fee is payable in full and is not recoverable from insurers.


9. Multiple Cancellations


If you cancel more than two consecutive appointments (whether self-funding or insured), we reserve the right to require that:

  • All future bookings are non-cancellable and non-refundable
  • Full pre-payment is made in advance
  • Payment is made via bank transfer only

Failure to comply may result in refusal of further appointments.


10. Late Arrival


Please arrive at least 10 minutes prior to your appointment.


If you arrive late:

  • Consultation time may be reduced accordingly; or
  • The appointment may need to be rescheduled

If rescheduling is required due to late arrival, the cancellation policy may apply.


11. Referrer / GP Communication


Unless you specifically request otherwise and it is clinically appropriate, we will inform your GP and/or referring clinician of your consultation and treatment.


12. Hospital and Third-Party Charges


Hospital, clinic, laboratory, pharmacy, and other third-party charges are separate from our professional fees.

These may include:

  • Room charges
  • Theatre fees
  • Nursing fees
  • Pathology charges
  • Pharmacy charges

You are responsible for settling these directly with the relevant provider.


13. AI Clinical Documentation (Heidi)


During consultations at Midland Skin, we use Heidi, a secure clinical documentation support tool which generates a real-time transcript to assist note-taking.

Key safeguards:

  • No audio recordings are stored
  • Data is processed and stored within the UK
  • Data is encrypted
  • Access is strictly limited to Dr Joanna Gach and Heidi acting as a contracted data processor
  • Data is used solely for delivering your care and managing medical records
  • Data is not used for marketing or unrelated product training

Heidi supports, but does not replace, clinical judgement.


By booking and attending an appointment, you agree to Heidi being used as a standard part of your care. Use of this system is a condition of care.


14. Data Protection


JG DERMATOLOGY LIMITED processes personal and medical information in accordance with UK GDPR and data protection legislation.


Information is stored securely and used only for purposes directly related to your clinical care and practice administration.


15. Complaints


If you are dissatisfied with any aspect of your care, please contact:


Complaints department at the medical facility you had your appointment.


Complaints will be handled promptly and fairly in accordance with professional standards.


16. Fees and Complaints


Professional fees are charged for clinical time, expertise, assessment, and services provided.

Payment for consultations and any procedures remains due in full once the service has been provided, regardless of:

  • Whether you are satisfied with the outcome of the consultation or treatment; or
  • Whether you subsequently raise a complaint.

Raising a complaint does not entitle you to withhold, delay, or refuse payment of fees properly charged under these Terms & Conditions.


Complaints will be handled separately in accordance with our Complaints Procedure.


Nothing in this clause limits your statutory rights or any entitlement to redress where we are legally required to provide it.


17. Late Payment and Interest


If payment is not made when due, we reserve the right to charge interest on the outstanding balance.

Interest will accrue:

  • From the due date
  • At a rate of 4% per annum above the Bank of England base rate
  • Calculated on a daily basis
  • Until payment is received in full

We may also charge reasonable administrative costs associated with recovery of overdue amounts.


This clause does not affect our right to pursue the principal debt through formal recovery procedures.


18. Debt Recovery


If an account remains unpaid after reasonable reminders:

  • The debt may be referred to a third-party debt recovery agency or legal representative.
  • You will be responsible for reasonable recovery costs, court fees, and associated legal expenses incurred in recovering the outstanding balance, to the extent permitted by law.

We reserve the right to suspend further appointments or services while any account remains in arrears.


19. Limitation of Liability


Nothing in these Terms & Conditions excludes or limits liability for:

  • Death or personal injury caused by negligence;
  • Fraud or fraudulent misrepresentation;
  • Any matter which cannot lawfully be excluded or limited.

Subject to the above, liability for services provided shall be limited to the extent permitted by law.


20. Acceptance


By booking and attending an appointment, you confirm that:

  • You have read these Terms & Conditions in full;
  • You understand your financial responsibilities;
  • You agree to be legally bound by them.
